Networking - Helping
the client and family understand and navigate the resources available
in the community.
Friendship and support - Emotional
support and companionship for the client and family by our trained
volunteers. Ongoing contact by telephone and home visit.
Respite Care - Providing limited
time off for the caregiver in need of a few hours away.
Help - Running errands and
providing light housekeeping.
Transitions is a program designed to help individuals
and their families cope with a life-limiting illness. The Transitions
Coordinator provides case management by helping the client and
family navigate servicess available in the community. Volunteers
offer friendship, respite care and emotional support. Together
the coordinator and volunteers provide a community of caring to
help enhance the client's quality of life.
Adapting to the changes that accompany a life-limiting
illness can be frustrating, confusing, and often frightening. It
can be challenging to keep one's independence while also learning
to rely more on others. Transitions can help empower you to make
choices that positively affect the quality of your life during
this difficult time.
- Needs help understanding information about their illness
- Is having difficulty doing housework or shopping for food
- Has occasional transportation needs
- Is feeling isolated or lonely
- Could use support for the caregiver in need of a break.
The Transitions Coordinator meets with cleints
and families to do an intake assessment, answer questions, and
to offer assistance. Following the visit, the coordinator will
refer clients and/or families to different community organizations
that can provide needed services. Volunteers are assigned to provide
assistance and support as needed.
To be eligible for Transitions, a person must
have a life-limiting illness of 12-18 months or less and approval
of their physicians. They may or may not be receiving active treatment
for their illness.
Because the Transitions services are provided
by a network of volunteers, there is NO CHARGE to clients and their
families.
Anyone can call the Transitions Coordinator to inquire. The Coordinator will contact the individual's physician to make sure he/she thinks the Transitions support is appropriate.
The Transitions program recognizes volunteers
as the cornerstone of the services to clients and families. A core
group of volunteers are recruited, screened, trained and supported
to assist in the most effective and skillful manner possible. The volunteers enable us to offer the program free of charge.
