When you need a helping hand: Adapting to the changes that accompany a life-limiting illness can be difficult, frustrating, confusing and sometimes frightening. Keeping one's independence, while learning to rely on others, can also be a challenge.
What is the Transitions program?
Transitions is a free support program designed to help individuals and their families navigate the challenges of a life-limiting illness. Our program coordinator provides education and case management by helping the client and family members to access and negotiate available community services. Volunteers offer friendship, respite care relief and emotional support. Together the coordinator and volunteers offer a community of caring and support to anyone who may still be seeking curative treatment for their disease.
Services provided by Transitions that helps to enhance a client's quality of life:
Networking - Helping the client and family understand and navigate the resources available in the community.
Friendship and support - Our trained volunteers provide emotional support and companionship for the client and their family. This support includes ongoing contact by telephone and home visits.
Respite Care - Providing limited time off for the caregiver in need of a few hours away.
Help - Running errands and providing light housekeeping, yard cleanup, locating donated durable medical equipment
In-Home Massage, Acupuncture, Foot Care (when funds are available) - Professionals make home visits to provide comfort and pain relief
Contact our Transitions coordinator today if any of the below issues apply to a patient or caregiver situation:
- Patient or caregiver has a need for help understanding information about an illness
- Patient is having difficulty doing housework or shopping for food
- Patient has occasional transportation needs
- Patient or caregiver is feeling isolated or lonely
- Caregiver could use support and is in need of a break
How are services provided?
The Transitions coordinator meets with clients and families to do an intake assessment, to answer questions, and to offer assistance. Following the visit, the coordinator will refer clients and/or families to different community organizations that can provide needed services. Volunteers are assigned to provide assistance and support as needed.
Who is eligible for Transitions?
To be eligible for Transitions, a person must have a life-limiting illness. They may or may not be receiving active treatment for their illness.
How much does it cost?
The Transitions services are provided by a network of volunteers; there is NO CHARGE to clients and their families. While there are costs associated with administration of the program, we depend on community donations for help rather than charging clients.
Who can refer to Transitions?
Anyone can call the Transitions coordinator to inquire. Referrals come from doctors, hospitals, social workers, family members, friends, and the clients themselves.
Why do we use volunteers?
At Redmond-Sisters Hospice, we realize that in serving our clients and their families, volunteers are the cornerstone of the Transitions program. As such, we recruit a core group of volunteers who are then screened, trained and supported to assist in the most effective and skillful manner possible. These volunteers, together with community donations, enable us to offer the program free of charge.
